How To Get Your Medical Card
We are a pharmacy where you may pick up your medical cannabis and have your Limited Medical Provider recommendations completed. We want to make the process move along as smoothly as possible so we've put together this helpful guide on how to get started!
Step 1
Know the qualifications.
The Department of Health has released a list of qualifying medical conditions. Speak with your medical provider to see if cannabis can elevate your way of life. (Spoiler: it will!)
Step 2
Meet with a Qualified Medical Provider.
All patients must meet with a qualified medical provider who has completed the State's mandatory training courses. Your medical provider will take note of your condition and can recommend cannabis to you.
Step 3
Create a Utah ID & apply for a medical card.
Review the EVS Patient Application User Guide and start an online application. After reviewing the guide, the patient may go online in the EVS and begin the medical cannabis card application process.
Step 4
Receive your QMP approval.
We recommend calling your provider after applying so your doctor knows they can approve the application.
*Difference Between QMP and LMP. A Qualified Medical Provider is a Utah-licensed medical practitioner that has undergone specific training in order to recommend medical cannabis to their patients. A Limited Medical Provider is any Utah-licensed medical practitioner with the authorization to prescribe medication to their patients. LMPs must be presented with the Limited Medical Provider Recommendation for Medical Cannabis Form and are limited to fewer patients than QMPs.
